If it's a web page /template issue, then do this:

Right click on the web page in question, view source, and at the top it 
will give the template name, then you can find the template in the 
surgemail/web folder and
add the options you want.

(And email me the patch and I will fix the master version, but I don't 
recall a credit card field off the top of my head so I'm not 100% sure 
what you are looking at...)
