Does this file exist:

/usr2/radxxxxxxx.com/zd/mi/mxxxxxx.mxxxxx/mdir/new/1464351184.16924_254215.sxxx

If so check it's ownership (and that of the rest of the path) to see if anything seems wrong (it should be owned by mail)
Try renaming it as user 'mail' and see if you can.
	su mail
	mv 1464351184.16924_254215.sxxx  1464351184.16924_254215.test
Does that fail?

My guess is it's owned by root and surgemail was started with the wrong ownership at some point in the past, if so
	chown -R mail /usr2/radxxxxxxx.com/
should correct things...

Failing that simple explanation then something really odd is going on...

We have one domain that rejects mail with a "maildb_rename failed” error.  Two examples with lots of x’s for the domain and mailbox names (and even our mail server's name).  The first is likely spam, but others weren’t, and the second is from the Mail Delivery Subsystem !

Failed localhost <> <dxxxxxxx.rxxxxxxx@radxxxxxxxx.com> 5938 <1464372013_7149@sxxx.xxxxxmail.net> "[localhost] from=["Mail Delivery Subsystem" <postmaster@radxxxxxxxxx.com>] maildb_rename failed (/usr2/radxxxxxxxx.com/jg/xi/dxxxxxx.rxxxxxxx/mdir/new/1464372013.16924_288435.sxxx Permission denied)"

These errors go back to before a surge mail restart.  They even go back before a server restart, but just now aware of them.  Other domains on the system are not showing this error.

Any ideas?
