Check your rules in surgemail.ini, the most common mistake is you've defined user access rules to limit what users can do via the web interface, and by doing that

if you miss certain keywords out of the list then spam filtering is also disabled.

Check this setting...

      g_user_access_default 
(But it could also be the user setting for access rules too in the user lookup page)
