I'm not quite sure I follow yet how you are setup.
- So the email address/account as surgemail is configured on is the published or the dummy unpublished address?
- And in terms of surgemail it is setup on the published or unpublished address?

Certainly setting the _user.dat pref_reply address should be getting used by surgeweb to generate the from header, and can be set to any address (much like a normal imap email client) if you make sure whatever you set it to matches allowed_from setting. If you are not using the multiple accounts feature that should be all there is to it, although you will struggle to eliminate all references to the real surgemail "account" in the headers I do believe it is possible with a variety less commonly used of settings.
The above situation may be complicated slightly by the use of the multiple accoutns feature in surgeweb, which at very least allows you to specify the reply-to header in addition to the above configuration. But may override the pref_reply as set in _user.dat I believe. To remove additional sources of confustion in your system you may want to remove the sw_identities..dat both from the surgeweb directory (tellmail swpathHIDDEN@omain) and surgemail xfile directory (tellmail pathHIDDEN@omain then subdir mdir/xfile).
So next step I suggest is send us (OFFLIST) the headers of the message in question from your sent folder in surgeweb and in its original form as it arrives at its destination and we should be able to offer further pointers.
Also which version of surgemail are you running?

In SurgeWeb, I see the From address as what I've specified and I can send out new email, but whenever it arrives, the from address is reverted back to my dummy account. The reply-to address is still myHIDDEN@ll.edu address, but the From header is still the private email address.
I can do this succesfully from Outlook if I connect up directly to Surgemail and just change the from address there, so I know Surgemail can do it, but I hope I'm not encountering a limitation in SurgeWeb. There's got to be a way to get it to change that header.
